School Physical Form for 2017-18 School Year
A current-year Pre-Participation Evaluation must be provided annually by:
1. All new students entering Cascia Hall for the first time,
2. All Middle School students because they take Physical Education classes, and
3. All Upper School students who plan to participate in extracurricular athletics.

This physical exam form needs to be completed by your personal physician after May 1, 2017. Completed forms are due to the school Registrar before practice begins or by August 1, 2017, whichever comes first.
Please make your doctor appointments early, as physicians are often inundated with requests for camp and school physicals all summer. Please take the school form with you to your appointments.
