Pre-Participation Evaluation Form

All students new to the school and all Middle School students must have a physical examination by their personal physicians after May 1 and before fall sports practice begins or August 1 (whichever comes first). All returning students who will be participating in an OSSAA sport or activity must also be examined by a personal physician annually. Please take the attached form with you to your doctor's appointment. This form is also available on the CH website Athletics page and Resource Board.
